**Job Overview** ---------------- The Operations Coordinator provides operational support for vendor management activities that support discharge planning, patient progression, and transition\-of\-care initiatives. Responsibilities include oversight of vendor resources within Epic, coordination of vendor relationships, workflow optimization, performance reporting, and resolution of operational barriers impacting patient transitions. This role serves as a key liaison between Children's Hospital Colorado and community\-based partners to support efficient, effective, and patient\-centered transitions of care. Additional Information Department Name: Case Management Job Status: Full time, 40 hours per week. Shift: Day, 8am to 4:30pm, No weekend or holiday requirements.
