* This positions is with the internal medicine residency program and bilingual is required in Spanish and English. **Position Summary:** Under the direction of the Ambulatory Care Manager, the Ambulatory Care Supervisor oversees the daily clinical operations, and patient care activities of assigned care teams. Within the supervisor role, will carefully balance direct patient care and supervisory duties to ensure the highest quality service and care is provided. Responsible for the delivery of patient\-centered services in accordance with the San Ysidro Health Model of Care designed to deliver safe, high\-quality, cost\-effective care, and improve health outcomes by meeting the population health management needs of the communities served. **Essential Functions of the Job:** * Supervise and coordinate daily team\-based functions according to the Patient Centered Medical Home care model.
