**This is a PRN position with the following pay rates:** * **Standard visits $75\.00** * **Evaluations $85\.00** The Physical Therapist employed by ALC Home Health is responsible for completing physical therapy services in adherence to all conditions outlined in the Personnel Service Agreement. The PRN Physical therapist position offers: * Flexibility and Convenience to control your own schedule and geographic area of your patients. * Administrative support to schedule and track visits utilizing Emergency Medical Response technology with direct communication to Home Health Director. **Essential Job Functions/Responsibilities** **1\.** Provides physical therapy services to patients according to a written physician's plan of care. This may include, but will not be limited to: * Assessing and evaluating therapeutic/rehabilitative/functional status, and participation in the development of the total plan of care.
