Location Fairbanks, AK, US Organization Name Clinical Services/Nursing Job Summary **Job Summary:** Transitions Case Managers ensure TCC patients have a smooth transition between different levels of care or healthcare settings. This role utilizes the nursing process and works collaboratively with the other members of the healthcare team to facilitate a seamless transition between hospitals, emergency departments, clinics, long\-term care facilities and home care. This role also serves as a liaison between our organization and outside facilities. The Care Transitions Case Manager reinforces the process’s that are set forth by TCC and the Indian Health Services in an effort to ensure outside entities can be successful in following those processes.
