**Field Medical Assistant – Patient Transitions \& Connected Care** **Remote Care Partners** **HealthyConnect \| East Tampa and Polk County, Florida** **Full\-time \| Weekdays \| $21–$24 per hour plus mileage and benefits** HealthyConnect is hiring personable, dependable Medical Assistants to visit patients in their homes after discharge from a hospital or skilled nursing facility. This is not a traditional office\-based Medical Assistant position. You will help recently discharged patients connect with their HealthyConnect clinical team and set up simple technology used to support their recovery at home. **What you will do** Complete approximately four to six scheduled home visits each weekday. Travel within a relatively compact assigned territory in East Tampa and Polk County. Deliver, install and test lightweight wireless health\-monitoring equipment. Teach patients and caregivers how to use the equipment. Confirm that readings are successfully reaching HealthyConnect’s centralized patient monitoring team.
