**Physical Therapist (PT) Home Health** **Location:** Oakland, CA \& East Bay Area **Job Type:** Contract **Contract:** 13 Weeks **Pay Rate: $ 1700 To $ 1900 per week** **Schedule:** Day Shift **Experience:** Minimum 1 Year Required **About the Role** We are seeking an experienced **Physical Therapist (PT) Home Health** for a 13\-week contract position covering Oakland and surrounding East Bay communities. The Physical Therapist will provide home\-based evaluations, treatment, reassessments, and rehabilitation services while traveling between patient locations. **Responsibilities** * Perform comprehensive home health physical therapy evaluations and develop individualized plans of care based on patient mobility, function, safety, and rehabilitation goals. * Provide skilled physical therapy treatments, reassessments, OASIS\-related visits, discharge evaluations, and other required home health services. * Educate patients and caregivers regarding therapeutic exercises, mobility, fall prevention, safety, assistive devices, and home exercise programs. * Complete timely and accurate clinical documentation, care coordination, and communication with physicians and other members of the int...
