**Job Description:** This position demonstrates consistent application of knowledge and skills to deliver appropriate, safe, efficient and cost\-effective pharmaceutical care within the scope of their assigned practice. This position is responsible for the care of patients as well as the development of effective, professional relationships within the multidisciplinary team. **This position will work 4, 10\-hour shifts rotating at LDS, Alta View, Riverton and the new Fairbourne Crossing facility.** **Essential Functions** * Reviews patients' medication order for allergy interactions, duplication of therapy, appropriateness of drug therapy, and drug\- drug interaction. Makes appropriate evidence\-based interventions to optimize medication use. Ensures safe, appropriate, cost\-effective therapies and accurately dispenses medications. * Oversees and/or participates in assuring appropriate medications are prepared and delivered timely to care areas and are stored both securely and appropriate for both stability and effective workflow. * Provides counseling to patients and promotes quality medication education and adherence.
