**Registered Nurse (RN) – GAPP / Pediatric Home Care** **TruCare 360 LLC** is seeking a dependable, compassionate, and experienced **Registered Nurse (RN)** to join our growing pediatric home care team. This position will initially involve providing skilled nursing services for a GAPP patient and providing nursing oversight and support for the patient’s caregiver. The RN will also assist with **new patient intakes, nursing assessments, care plans, caregiver training, and ongoing clinical needs** as additional GAPP patients are accepted. **Current Case –** The initial assignment will involve working with a pediatric GAPP patient currently approved for approximately **8 hours of skilled nursing services per week**. The RN will: * Provide skilled nursing services according to the patient’s approved plan of care. * Complete and maintain required nursing documentation and visit notes. * Monitor the patient’s condition and report significant changes as appropriate. * Develop, review, and update the nursing care plan as needed. * Provide caregiver education and training. * Provide nursing oversight and guidance to Braxton’s mother, who will be providing the patient’s approved personal car...
