Summary The Client Care Coordinator will be responsible for overseeing the delivery of high\-quality home care services across multiple counties and various contracts. This role is pivotal in ensuring that clients receive tailored care plans, facilitate communication between clients, families, and healthcare providers, and manage the logistics of service delivery. Job Responsibilities * New Client Intake Responsibilities o Inputs all new clients into AxisCare, ensures billing entered, updates annual service agreement dates to ensure no lapse in services. o Process incoming client referrals and document new client information in AxisCare. o Collaborates with Scheduler, HR, and DOO to discuss rising care needs and conflict resolutions. o Completes customer service “Rising Star Surveys,” follows up with new clients to ensure satisfaction, provides * Coordination of Service o Manage client scheduling activities and notify clients of changes, call\-offs, or relevant updates. o Coordinates Service plan needs with Scheduler o Liaise with healthcare providers, family members, and clients to facilitate communication and address concerns. * Contract Management o Oversee compliance with vario...
