**We are looking for a dedicated candidate join our team as a Community Health Worker (CHW) to service our patients in the Savannah area.** The Community Health Worker (CHW) serves as a vital link between patients, healthcare services, and community resources, ensuring seamless access to care and improving the quality\-of\-service delivery. This role includes visiting patients in their homes and Assisted Living Facilities (ALFs) to facilitate visits with providers and specialty services.
