**General Statement of Job:** The Care Manager Extender will report to the Tailored Care Manager and with providing whole person care to consumers. Care Manager Extenders will perform functions within their training, content, and abilities. The Care Manager Extender is in regular communication with supervising Care Managers and engage in coordinating efforts to ensure consumer enrollment and engagement in Tailored Care Management Services through direct care/contact. Candidate may work from our Winston\-Salem or Greensboro location, depending on the staffing needs in each area. **Qualification and Education Requirements:** It is required that the applicant of the role must be at least 18 years of age and must have obtained at minimum a High School Diploma /GED. Applicants of the role must also meet at least one of the below requirements. Be a person with lived experience in working with individuals with Mental Health, Substance Use and/or I/DD diagnosis. Must have demonstrated knowledge of and direct personal experience navigating the North Carolina Medicaid delivery system.
