Easterseals is seeking an experienced and motivated School\-Based Physical Therapist Coordinator to lead and support physical therapy services in a school base setting. This role is ideal for a licensed Physical Therapist who thrives in a leadership role and is passionate about helping children succeed in a school\-based setting. What You’ll Do * Conduct initial and ongoing evaluations, maintaining accurate documentation and progress notes. * Develop individualized, goal\-driven treatment plans and reassess students according to established timelines. * Collaborate with interdisciplinary teams to optimize scheduling and maximize therapeutic outcomes. * Oversee scheduling for evaluations, referrals, and screenings. What You Bring * Active Illinois Physical Therapist license. * Minimum of 2 years of experience in a school\-based physical therapy setting. * Current Child CPR and First Aid certification (or willingness to obtain). * Prior supervisory or leadership experience preferred. * Strong organizational, communication, and collaboration skills.
