**Position Summary:** Responsible for tracking and follow up of clients in order to maintain an 80% follow up rate. Responsible for the administration of admissions, discharge and follow up assessments. Responsible for other program evaluation activities. Electronic Record Control\- Enters knowledge assessments and other project data into the computer system. Assist Project Supervisors or Project Coordinator with quality assurance processes. **Essential Job Functions:** * Develop and maintain basic statistical databases as needed * Collect quantitative and qualitative data utilizing the data collection worksheets. * Conducts admission, discharge, and follow\-up evaluation assessments for clients. * Maintains system to identify timelines for individual client evaluations and ensures that evaluations and updates are conducted within designated timeframes. * Tracks clients to ensure availability for ongoing assessments, including developing outreach plan and incentive structures * Enter program\-specific client data in the statistical databases * Enters data including: * GPRA data into SAIS system * Continuum of Care Web\-based Homeless Reporting System * Other data collection tools a...
