The Patient Intake Coordinator plays a critical role in transitioning patients from referral to care by conducting initial outreach calls, gathering and verifying required information, explaining Personic Health’s services and care process, and scheduling the patient’s first visit. This individual must communicate clearly, compassionately, and confidently, directly influencing referral conversion, patient satisfaction, and timely access to essential wound care.
