Promotes rational medication therapy by evaluating all orders in the context of current literature, UAB medication evaluations and the UAB Formulary, contacting the physician or healthcare professional and making recommendation for changes when appropriate. * Actively monitors patient specific response to prescribed therapies; reviewing potential adverse drug reactions and drug interactions to optimize therapy.
