You will play a critical role in improving patient outcomes through effective medication management, patient education, and collaboration with our healthcare team. In this role, you will support the full physician panel, regardless of health plan coverage and may involve multiple clinic locations. In this remote position, you will report to the Associate Director Integrated Clinic Pharmacist and will integrate the pharmacist as a member of the care team within the physician practice. Your daily responsibilities will include: * Use evidence\-based medicine and clinical judgment to evaluate the suitability of pharmacotherapy for high\-risk patients with chronic conditions, including diabetes, hypertension, heart failure and COPD. * Execute clinical protocols as allowed per organization and state regulatory agencies. * Optimization of drug coverage benefits for appropriate care and reduced costs (preferred formulary products, lower cost alternatives and home delivery pharmacy). * Collaborate with physicians, nurses, and other h...
