**Overview** We are seeking a dedicated and compassionate Registered Nurse to join Comfort Heart Hospice Case Management team. Primary functions are to perform admissions and administer nursing care for patients in their place of residence or facilities, coordinate are with the Interdisciplinary Group/Team, patient/family and referring agency; and assume the responsibility for coordination of care. The ideal candidate will possess a strong commitment to patient care, with the ability to work effectively in various clinical settings. This role requires a comprehensive understanding of nursing practices, medical terminology, and patient assessment techniques.
