**Job Description** Safe Life Home Health is seeking an office based Clinical Case Manager to support referral intake, case triage, coordination of care, and communication with patients, families, referral sources, and the clinical team. This role is ideal for an RN, LPN, or LVN who is organized, responsive, and comfortable managing a high volume of incoming cases in a fast paced home health setting. We now serve patients through two branches: * Lombard branch with a 4\.5 stars Medicare Quality Rating * Chicago branch with a 5 stars Medicare Quality Rating The Clinical Case Manager will play a key role in reviewing incoming referrals, triaging cases appropriately, helping determine eligibility and service needs, coordinating with intake and clinical staff, and ensuring smooth case flow from referral through admission readiness. **Pay Range** $37 to $42 per hour **Key Responsibilities** * Review and triage incoming home health referrals in a timely manner * Coordinate with referral sources, hospitals, physicians, discharge planners, and internal staff to gather required documentation * Assess referral appropriateness based on clinical needs, payer requirements, service area, and sta...
