**Physical Therapist (PT) Home Health** **Location:** Rutland, VT **Job Type:** Contract **Contract:** 13 Weeks **Pay Rate: $ 2,000 To $ 2,279 Per week** **Schedule:** Day Shift **Experience:** Minimum 2 Years Required **About the Role** We are seeking an experienced **Physical Therapist (PT) Home Health** for a 13\-week contract in Rutland, Vermont. The Physical Therapist will provide comprehensive home\-based rehabilitation services, including patient assessments, individualized treatment planning, skilled therapy interventions, patient and caregiver education, and coordination of care with the interdisciplinary healthcare team.
