Overview: We are seeking a licensed Physical Therapist to provide comprehensive, individualized rehabilitation services in a skilled nursing facility. This role combines independent clinical evaluation and treatment planning with direct resident care, interdisciplinary communication, and oversight of delegated services. The successful candidate brings strong clinical reasoning, dependable documentation practices, and a commitment to measurable functional outcomes. Clinical Evaluation \& Plan of Care * Review physician referrals/orders, clinical history, precautions, prior level of function, and relevant diagnostic information before evaluation. * Perform and document initial evaluations and medically indicated reevaluations, including range of motion, strength, balance, endurance, pain, gait, transfers, mobility, and fall risk. * Identify rehab potential, functional limitations, reasonable measurable goals, appropriate frequency and duration, and anticipated discharge needs. * Develop, establish, and revise individualized physical therapy plans of care within applicable professional and payer requirements. * Determine when skilled physical therapy is indicated, when a resident shou...
