The Care Coordinator will assist high risk, complex clients to access needed health services and complementary social services. The Care Coordinator will coordinate services and community support to achieve improved health outcomes for clients. **Hourly Rate: $30\.77 \-36\.71 DOE** **Duties and Responsibilities:** * Review daily referrals; meet with clients in order to obtain program consent and review client’s care plan, medical follow\-up and psychosocial needs. * Initiates contact and establishes a working relationship with patients for the purpose of providing assistance with attaining needed health and social services. * Assist client in establishing individualized health goals, identifying potential problems/barriers; provide health education resources. Provide advocacy for client and monitor client’s progress. Coordinate logistics for health goal adherence. * Assist in timely scheduling of primary care and other medical appointments to support client health.
