**Job Description:** This position demonstrates consistent application of knowledge and skills to deliver appropriate, safe, efficient and cost\-effective pharmaceutical care within the scope of their assigned practice. This position is responsible for the care of patients as well as the development of effective, professional relationships within the multidisciplinary team. **The position will mainly staff the cardiology floors from 7am to 3:30pm Monday through Friday. The position will also rotate through weekend shifts with an approximate 1 in every 5 weekends. In addition there is a week (5 days) of 12 hour shifts 7am to 7pm that occurs every 6 weeks where the position will staff in our Thoracic ICU. Besides those long shifts there are not evening or overnight shifts required of this position.** **Essential Functions** * Reviews patients' medication order for allergy interactions, duplication of therapy, appropriateness of drug therapy, and drug\- drug interaction. Makes appropriate evidence\-based interventions to optimize medication use.
