**Home Health Physical Therapist Opportunity!** **Monday\-Friday, 8a\-4:30p** **Full Time or Part Time** ***Family Home Health*** is seeking a **Physical Therapist** who is ready to make a difference in the lives of older adults. We believe that every staff member who comes in our doors is a part of our family. **Essential Functions include but are not limited to:** * Participate in the development of the total plan of care. * Maintain appropriate clinical records, clinical notes, and report to the physician any changes in the patient’s condition.
