**JOB PURPOSE:** Provides pharmaceutical services to patients receiving care in post\-acute, long term, or retail settings in a manner that delivers superior patient care and excellent customer service. Monitors and manages the prescription process and daily activities of the closed\-door long\-term care pharmacy in accordance with company, state and federal policies, laws and regulations. **ESSENTIAL JOB FUNCTIONS, DUTIES, AND RESPONSIBILITIES:** * Review prescriptions issued by prescribers to assure accuracy and determine formulas or ingredients needed. * Evaluate prescriber medication orders for appropriateness of drug, dosage, potential interactions, route of administration and stability. Make recommendations for medication therapy changes to prescribers as appropriate. * Collaborate with the healthcare team to provide clinical and technical support as needed. * Document all communication with physicians, nurses, and other members of the healthcare team. * Take responsibility for accurate intake and efficient fulfillment of prescription orders. * Obtain, as necessary, and maintain all required documentation in accordance with federal and state regulatory agencies and Pharmacy S...
