The Physical Therapist reviews the plan and goals and progresses and/or revises the program appropriately. * Evaluates patient and develops goals and appropriate treatment plan. * Implements and coordinates Physical Therapy treatment within the overall treatment plan, coordinating care with other professionals as needed. * Reviews plan of care and progresses or adjusts program as indicated and according to department policy. * Informs, counsels and consults with patient's family, case managers, industries, etc. and other health care workers. * Documents patient data (evaluation, treatment and progress notes, D/C summary, charges) and department statistics according to department policies and procedures. * Provides/recommends adaptive equipment or assistive devices as indicated to increase the independence of the patient. * Plans di...
